The standard 6 core biopsy, with an accuracy about 16%, has been replaced by 12 and 24 needle cores, improving the diagnostic rate. 1/3 of tumors will occur deeper than the normal thrust of the guide and will be missed unless the sonogram or MRI demonstrates a remote lesion (which are not palpable by the DRE as they lie far from the examining finger.) Biopsies do leave cancer cells in the needle tract which may also enter the blood stream producing tumors in distant organs.
MRI is the current gold standard but is less accurate in the presence of prostatitis (a common low grade infection) that has few symptoms. Sonograms predicting this inflammation can prevent misdiagnoses.
Radical surgery is time honored and frequently displays more aggressive tumors than predicted by earlier biopsies. Side effects are impotence and incontinence.
Recurrence may occur in up to 20% of high grade tumors.
